Speech Delay in Toddlers: When to See a Doctor

Speech Delay in Toddlers: When to See a Doctor

In short: Most children say their first words around 12 months and join two words by 2 years. See a pediatrician if your child isn’t babbling by 12 months, has no words by 16 months, no two-word phrases by 2 years, or loses words they used to say. Early help makes a real difference.

Speech milestones (approximate)

AgeWhat most children do
2–3 monthsCoos and smiles back at you
6 monthsBabbles (“ba-ba”, “da-da”), turns to sounds
9–12 monthsUnderstands “no”, waves bye-bye, points; first word around 12 months
18 monthsSays several single words, follows simple instructions
2 yearsUses 50+ words and puts two words together (“more milk”)
3 yearsSpeaks in short sentences that family members mostly understand

Children growing up with two or more languages — common in Bangalore homes — may mix languages or start a little later, but they should still reach milestones in their combined vocabulary. Bilingualism does not cause speech delay.

Red flags — see a pediatrician

  • No babbling by 12 months, or no pointing or waving
  • No single words by 16 months
  • No two-word phrases by 2 years
  • Losing words or skills your child already had (at any age)
  • Not responding to their name, little eye contact, or not interested in people
  • Speech that even family can’t understand by age 3

Common causes of speech delay

  • Hearing problems — including fluid in the ears after repeated ear infections
  • Expressive language delay — the child understands well but talks late
  • Not enough conversation time, including high screen use
  • Developmental conditions such as autism spectrum disorder or global developmental delay
  • Problems with the mouth or tongue movements (less common)

What happens at the visit

Your pediatrician will ask about milestones and behaviour, examine your child, and check how well they understand and interact. A hearing test is usually the first step. Depending on what we find, we may refer your child for speech therapy or a developmental assessment. Starting therapy early — even before age 2 — often gives the best results.

Simple ways to help at home

  • Talk through your day: name things as you cook, bathe and dress your child.
  • Read together every day, pointing at pictures and naming them.
  • Pause and wait — give your child a chance to respond before you speak again.
  • Expand their words: if they say “car”, you say “red car” or “big car”.
  • Limit screens: none under 2 years apart from video calls; under an hour a day of good content, watched together, for ages 2–5.
  • Sing rhymes and songs in your home language.

If you’re worried, trust your instinct — it’s always better to check early. Learn about our growth and development checks or book a visit.
